What to expect DURING the physical

At DOCS, we take your family’s medical history and cross-reference that with the vitals, hearing, vision and other conditions your physician will check on. This is just in case there is any conflicting information that could prevent them from participating in any fun. We will also run any additional tests as needed at our Monroe location.

Physical Examination Includes:

  • Height / Weight
  • Blood pressure and heart rate
  • Checking of heart, lungs, abdomen, ears, nose, and throat
  • Evaluate posture, joint, strength, and flexibility
  • Vision testing
    *Concussion Assessment
    *Recommendation of Proper Protection
    *Recommendation to Specialist if Necessary

Medical History Includes:

  • Serious illnesses among other family members
  • Illnesses (past and present) due to asthma, diabetes, or epilepsy, etc
  • Previous hospitalization or surgeries
  • Past injuries (including concussions, sprains, or bone fractures)
  • History with passing out, dizziness, chest pain, or trouble breathing during exercise.
  • Medications that you are currently on (over-the-counter/ herbal supplements/ prescription.)
  • Vaccinations

What should I bring to the physical?

Any required forms our healthcare providers need to fill out can be brought to DOCS Urgent Care. These forms are typically from the school, camp or program and are usually necessary to participate in it.

In addition, you should bring:

  • A valid photo ID and insurance card for the guardian
  • Any current prescriptions for the child or teen, along with any glasses or contacts.

Who needs Sports or Camp Physicals?

Updated physicals will ensure that your child will be able to participate with their recreational or competitive Monroe sports team.
