EFMCSA medical standards evaluate:

Vision: At least 20/40 acuity in each eye (with or without correction), adequate peripheral vision, and the ability to recognize traffic signal colors

Hearing: Ability to hear a forced whisper from at least five feet away (with or without a hearing aid)

Blood Pressure: Readings below 140/90 for two-year certifications; higher readings may result in shorter certification periods or disqualification

Diabetes: Controlled blood sugar with HbA1c documentation; insulin-dependent diabetes requires additional evaluation

Cardiac Health: No current cardiovascular conditions likely to cause sudden incapacitation

Sleep Disorders: Absence of untreated sleep apnea or other conditions affecting alertness

Medications: Current prescriptions must not impair driving ability or reaction time

Physical Capability: Adequate strength, mobility, and coordination to safely operate commercial vehicles
