Who Needs an Immigration Physical in Connecticut?

Anyone applying for a U.S. green card or adjustment of immigration status is required to complete an immigration physical, also known as a medical exam. If you’re living in Connecticut and filing Form I-485 with USCIS, you’ll need to have this exam performed by a USCIS-designated civil surgeon to ensure you meet the health-related requirements for immigration.

Before Your Appointment

Once you schedule your immigration physical at DOCS, download and review Form I-693 (Report of Medical Examination and Vaccination Record). Complete your personal information in Part 1 before your appointment. Please also gather your medical records, especially your vaccination history, and bring them with you.

On Appointment Day

Please bring the following to your appointment:

Please register at the front desk, where our staff will confirm that you are scheduled to see a USCIS-designated civil surgeon. You’ll fill out a short health questionnaire to help the doctor understand your current and past medical conditions, medications, and allergies.

After Your DOCS Immigration Physical in Connecticut

The immigration physical is often completed in one appointment. In some cases, however, you’ll need to return to our office, most commonly to receive additional vaccinations or have your PPD skin test for tuberculosis interpreted.

Once your physical is complete, our civil surgeon will finalize your Form I-693, including any required lab results and vaccine updates. The completed form will be placed in a sealed envelope, which you must not open. The envelope is returned to you or submitted directly to USCIS, depending on your preference.
