What to expect DURING the physical

To ensure that your child is healthy and can physically participate in a sport or camp activities, we will check their vitals, flexibility as well as vision. By checking these areas, we’ll be able to test your child’s limitations in physical activity and diagnose any underlying conditions you may not have known about. For your appointment, we recommend that you bring along your child’s vaccination records and let us know about your family’s medical history.

Physical Examination Includes:

  • Height / Weight
  • Blood pressure and heart rate
  • Checking of heart, lungs, abdomen, ears, nose, and throat
  • Evaluate posture, joint, strength, and flexibility
  • Vision testing
    *Concussion Assessment
    *Recommendation of Proper Protection
    *Recommendation to Specialist if Necessary

Medical History Includes:

  • Serious illnesses among other family members
  • Illnesses (past and present) due to asthma, diabetes, or epilepsy, etc
  • Previous hospitalization or surgeries
  • Past injuries (including concussions, sprains, or bone fractures)
  • History with passing out, dizziness, chest pain, or trouble breathing during exercise.
  • Medications that you are currently on (over-the-counter/ herbal supplements/ prescription.)
  • Vaccinations

What should I bring to the physical?

Before you come in for your child’s sport or camp physical, be sure to bring their required forms for the school, camp or program they plan to participate in. We would also like to see your child’s current prescription as well as their glasses or contacts. A parent or guardian must also bring a Photo ID and current insurance card.
