What to expect DURING the physical

Our physicians will check your athlete’s vitals, flexibility as well as vision to help diagnose and limitations on their physical activity. We also recommend that you bring along your child’s vaccination record and inform us on your family’s medical history to see if we need to recommend additional tests.

Physical Examination Includes:

  • Height / Weight
  • Blood pressure and heart rate
  • Checking of heart, lungs, abdomen, ears, nose, and throat
  • Evaluate posture, joint, strength, and flexibility
  • Vision testing
  • Concussion Assessment
  • Recommendation of Proper Protection
  • Recommendation to Specialist if Necessary

Medical History Includes:

  • Serious illnesses among other family members
  • Illnesses (past and present) due to asthma, diabetes, or epilepsy, etc
  • Previous hospitalization or surgeries
  • Past injuries (including concussions, sprains, or bone fractures)
  • History with passing out, dizziness, chest pain, or trouble breathing during exercise.
  • Medications that you are currently on (over-the-counter/ herbal supplements/ prescription.)
  • Vaccinations

What should I bring to the physical?

Whether you schedule your appointment or come for a walk-in visit, you will need to bring the required forms from the school, camp or program for our healthcare providers to fill out. We would also like to see a record of your child or teens current prescriptions as well as their glasses or contacts. Parents or legal guardians must also bring a Photo ID and current insurance card to the visit.
